Tool to Help People with Advance Directives for Their End-of-Life Care Gets Increased Distribution Through Employers
Staff and consultants with the Commission on Aging with Dignity, a nonprofit organization in Tallahassee, Fla., revised and disseminated the Five Wishes advance directive, which is designed to help people write down their wishes about end-of-life care and discuss them with family members and physicians.
The project was part of the Robert Wood Johnson Foundation's (RWJF) national program Targeted End-of-Life Projects Initiative.
Key Results
Project staff and consultants revised the Five Wishes advance directive document to meet the legal requirements of 33 states and the District of Columbia.
More than 1,400 individuals and groups requested and received more than 400,000 copies of Five Wishes for distribution.
About 500 hospitals, places of worship, long-term care facilities and senior centers held Five Wishes workshops to explain how to use the document.
Project staff and consultants updated and redesigned the Five Wishes document and developed an employer packet to assist with distribution in the workplace.
Project staff recruited at least 500 employers who distributed more than 500,000 copies of Five Wishes.
